Did anyone else notice how weird and wrong page 104 is?

I had one nagging question throughout the latter part of the book. The author wrote quite a bit about how "patient zero" or the "index case" of this particular outbreak of Cholera was identified, but there was nothing about how this particular patient contracted Cholera in the first place. Did I miss something?
