This question comes up often and as so many of us listen to audiobooks I thought it might be helpful to provide a permanent place for the instructions.
I save the audiobook file to my desktop/laptop. Then I open iTunes, go to the file menu and select "Add file to my library" or "Add folder to my library" depending on how the audiobook file is saved.
Once added, most of the audiobook files come in to iTunes defined as music files. Go to the music library, right click on the file (you can search for it by the author or book name), select "Get Info" and click on the options tab. (If there are multiple files, select all of them first before you "Get Info".) Change "music" to "audiobook" and check the two options below (I can't remember them offhand but I believe one excludes it from shuffle and the other remembers start and stop points). I also change the equalizer to "spoken word" and set the volume at 100%. Doing that eliminates that up and down sound modulation you can sometimes encounter.
You should also click the sort tab before saving. Go in and fill out the fields for title and author so the book shows up in your audiobook library correctly.
Save and voila! It will now appear in the iTunes audiobook library.
